Please consider Flu vaccines-it's not too late. Please remind family members to wash their hands, it's the number way to prevent the spread of germs. Cover your mouth with arm when coughing or sneezing. If you have any medical concerns regarding your child please contact the nurse. Starting this year medications must be brought to the nurse by a parent/guardian, students are not to bring medications to school.
If your child needs medications to help during allergy season, or cough drops, please have a parent/guardian drop medications to the nurse with a note from parent if over the counter and a note from the doctor if prescription. If your child takes a prescription medication, a parent/guardian must bring it in the first day of school with a current doctor note and parent note for the school year.*Important: For the start of the 2018-19 school year all students must have proper immunizations by the fifth day of school or there is a risk of exclusion per new Pennsylvania legislation. Please check with your child's health provider to ensure that they have documentation by the first day of school 2018-19.
Please remember to send in the following state mandated forms to the Health Suite:
Kindergarten/First: Physicals and Dentals
Third grade: Dental forms
6th Grade: Physical and Scoliosis forms
Anytime your child has immunizations/vaccinations please request a copy of what your child received and send to School Nurse.We accept Physical and Dental forms that are due from August of the previous school year
Please note that you can obtain copies of Medical, Dental, Medication forms on the npenn.org website under for families and then health forms.You can obtain a copy of the Physical and Dental forms at (www.npenn.org). Click on “For Families” then click on “Frequently Used Forms”.
