     Welcome to Gwyn Nor Elementary Health Suite! Please review the following it includes Pennsylvania state requirements for school students.

    Reminder to get Flu vaccine for students and families. Also, if a student has a fever of 100.5 or greater they must be picked up, students should be fever-free without medications for 24 hours before returning to school.

     If your child needs medications to help during allergy season or cough drops, please have a parent/guardian drop medications to the nurse with a note from a parent if over the counter and a note from the doctor if prescription medication. If your child takes a prescription medication, a parent/guardian must bring it to school with a current doctor note and parent note for the school year. Students are not permitted to transport medications to and from school.

    *Important: For the start of the 2019-2020 school year all students must have proper immunizations by the fifth day of school or there is a risk of exclusion per new Pennsylvania legislation. Please check with your child's health provider to ensure that they have documentation by the first day of school 2019-20. 

    Please remember to send in the following state-mandated forms to the Health Suite:

    Kindergarten/First: Physicals and Dentals

    Third grade: Dental Exam forms

    6th Grade: Physical Exam  forms

    Anytime your child has immunizations/vaccinations please request a copy of what your child received and send the updated information to the School Nurse.

    We accept Physical and Dental forms that are due from September of the previous school year

    Please note that you can obtain copies of Medical, Dental, Medication forms on the npenn.org website under for families and then health forms.

    You can obtain a copy of the Physical and Dental forms at (www.npenn.org). Click on “For Families” then click on “Frequently Used Forms”.
