Kindergarten Registration

For Grades 1-12
Students entering grades 1-12 in the 2013-2014 school year can be registered throughout the summer. Appointments can be made online, by clicking here. Please scroll down to the bottom of the page to obtain paperwork and documents that are necessary for your appointment.

New Student Registration Information – for the current school year.

Attention parents/guardians: Please click here to schedule an appointment online to register new students for the current school year. Please remember to complete all of the necessary paperwork listed below on this webpage. The paperwork will be required at your appointment.

In order to make the registration of your child as easy as possible, your assistance is required. The Commonwealth of Pennsylvania and school district policy requires written documentation in the following areas prior to your admittance to school.
 
* Please print the forms underlined below. Complete them and bring them with you to your registration appointment.
 
*If you are registering a new kindergarten student you do not need to print the request of student records form or the discipline record form.
 
Registration Forms

 

Proof of Residency

You will need to bring a copy of an agreement of sale, settlement papers, mortgage form, current utility bill or other official document showing your address as it is to appear on the registration.
 

Proof of Child’s Age

You will need to bring a copy of your child’s birth certificate, church record, physician’s statement or previous school record to verify age. Child must be 5 years of age by September 15, 2012. School board policy does not allow for any exceptions to this date.
 

Proof of Immunization

You will need written documentation (month, day, and year) from a hospital or clinic, physician or previous school immunization record which verifies that your child has received the following immunization. Please note that the immunization grid below has been recently updated as of October 14, 2010.

These immunizations may be completed by your family physician, the North Penn Visiting Nurses Association, 51 Medical Campus Drive, Lansdale, PA, 19446, 215-855-8296, or the Montgomery County Health Department, 610-278-5145. If you have religious or philosophical objections to any of the immunizations, please advise us of this at registration. If there are medical reasons your child is not vaccinated, a statement from a licensed physician giving reasons for exclusion must be filed in the student’s record.
 

School Transfer Card, Current Report Card or Official/ Unofficial Transcript

You will need to bring one of these documents if your child previously attended another school. If your child has participated in a special education program, his/her previous school district a copy of the Individualized Education Placement (IEP) is most helpful.
