Each long term problem statement includes a section titled "The Team Must Provide". The required paperwork is listed here; included are four copies of the Style Form, one Cost Form, one Outside Assistance Form and four copies of the Team List. These forms can be found in the Member Area of the national OotM website (www.odysseyofthemind.com). For competition, I would advise putting a completed copy of all forms in two separate folders; keep one with you and give the other to your co-coach or another adult associated with the team for safe keeping.

Thoughtful preparation of all forms is essential but because judges use the Style and Team List Forms to identify scoring elements, these forms should be given careful attention. Answers on these forms should be clear/ concise and should include all the information the judge needs to score each requirement. The team should start drafting their forms as soon as possible and should review/revise the forms before the final draft is prepared. The judges will also talk to the team after the performance to ascertain if requirements were met. During this time, team members should be prepared to share details about how they've met the requirements (same as noted on the forms). The team should realize that these forms and the post-performance interviews with the judges are opportunities to tell the judges how well they've met the requirements of the problem.

Long term and style judges have up to eight minutes to identify and score problem requirements during a team's performance. If the team submits carefully worded forms and prepares for the post-performance 'interview', the judges are better able to assess the team's performance.
